About carbamazepine
What it treats
Carbamazepine prevents seizures in epilepsy, particularly focal seizures, and treats trigeminal neuralgia. It is also used in some bipolar disorder treatment. It is on the World Health Organization Model List of Essential Medicines.
Why it is widely used
It has been a first-line epilepsy medicine for decades, is inexpensive, and is bought by public health systems in large volumes. Programmes that treat epilepsy at primary care level rely on it.
How it is taken
It is taken two or three times a day, starting at a low dose that is raised slowly. It must not be stopped suddenly, because that can bring on seizures or withdrawal effects. It interacts with many other medicines, including hormonal contraceptives, and can cause dizziness and low sodium. In people of some Asian ancestries a genetic test is advised before starting, because of the risk of a severe skin reaction. It is avoided in early pregnancy unless the benefit outweighs the risk. Patients should always follow the approved label and the prescriber's instructions in their country.
